Show a genuine stack trace from an incident you have had. Ask what they see and what they would check first. Strong candidates narrate a search: which frames are yours, which are framework, what the exception type rules in and out. Weak candidates read it aloud. “Two threads write to the same cache. Reads occasionally return stale data under load, never in testing. Step 5: Replace the take-home with a 90-minute code-reading session
This is the change that came directly out of the outage. The job is mostly reading and safely changing code other people wrote. So test that.

It usually means one of two things: the candidate worked on systems with no production exposure, or they were never trusted with the response.
